Pricing
$275 per adult/child for exam and form completion
$70-$110 for required blood tests
$30-$100 for additional titer testing (if needed)

What To Bring
1- Form of ID: A valid government-issued photo identification document (passport/greencard/driver’s license/visa, etc)
2- Vaccine Record: If available, please bring in any copies of foreign or domestic immunization records. If you don’t have it we can provide you with a blood test order to determine your immunization status (titer testing).
3- I-693 Form: Please fill out the first three pages of the form and your name at the top of each page. DO NOT date or sign the form until instructed to do so. If you do not have the form or are unable to bring it, we will provide you with it at the day of your appointment.

Required Blood tests
Tuberculosis: all applicants age 2+ must have interferon gamma release assay (IGRA) blood testing. If positive, you will need a Chest X-Ray.
Syphilis: required for applicants age 18-44
Gonorrhea: required for applicants 18-24
No immunization record?
Laboratory evidence of immunity is acceptable for some of the immunizations required. We will order the blood tests to check for immunity to these infections.
*Note: USCIS will only accept titer testing ordered by the civil surgeon. Prior records will not be valid.
Vaccinations will be recommended based on USCIS guidelines.
We do not offer vaccinations in the clinic.
Most vaccines can be taken at the pharmacy or the public health department.
You can use your insurance to get vaccines.
We do not recommend getting the vaccines done prior to the appointment. Vaccine requirements may change based on updates from USCIS. We will provide you the most up to date information on what is required.
Final Report
The Final Report will typically be ready in 1-2 weeks after the appointment date if all requirements are met.
You will be notified when the report is ready to be picked up.
We will release the original form in a sealed envelope and provide you a copy for your records.
